The Marley Medical clinical team will be available to answer your questions until Aug 31, 2024. You can reach us through the patient portal or by calling the clinic at 888.890.2008.

Starting Sept 1, 2024, you will not be able to reach the Marley Medical care team.

I need a refill. How do I get one?

If you require prescription refills before transitioning to a new provider, please contact our office as soon as possible and before Aug 31, 2024. We will ensure that you have the necessary medications to carry you through until you have established care with a new provider.

We will not be able to provide prescriptions or refills after Aug 31, 2024.

How do I get access to my health information? Will you send it to my doctor?

Your medical records are confidential and will remain so even after our practice closes.

If you would like us to send your medical records to another healthcare provider, please let us know their name and fax number before the last week of August. We will then electronically fax your records to their office.

I need to find a new doctor. Can you recommend any?

Devices

Can I keep using my device?

Yes, you can continue to use your Marley Medical provided devices, like a blood pressure monitor, glucometer, and scale, after the clinic has closed. Your care team will no longer be monitoring your data after Aug 31, 2024 but you may use your device(s) for personal monitoring. We strongly recommend seeking care from a new healthcare provider to help you manage your health.

How do I get more Glucometer supplies?
